Hypertension RN, PD - Northern Manhattan / Bronx - Bilingual Spanish/Engl Req'd

PURPOSE OF POSITION
Responsible for establishing standards of care for the Hypertension Home-Based Support Research Program for African Americans. Educates, counsels, and provides information on hypertension and treatment to VNS patients who have high blood pressure and have agreed to participate in this study. Also provides on-going evaluation of patient's anti-hypertension medication regimens. Assists patients in adhering to medical and behavioral regimens and facilitates their effective self-care management. Works under general supervision.

RESPONSIBILITIES

Identifies, develops and implements policies and procedures establishing standards of care for the home support program. Assists Research Center staff in determining the best data collection tools for the project and designing electronic record review protocols.
Trains, oversees and manages the Community Health Educator. Trains and oversees the research interviewers on use of the Blood Pressure device.
Works with Research Center to provide comprehensive counseling to VNS patients with high blood pressure. Provides home visit(s) and ongoing telephone support to patients enrolled in the Hypertension Support Program.
Educates and provides information to patients on hypertension and treatment. Contacts study participants via telephone in the evenings and weekends for ongoing management, as necessary.

QUALIFICATIONS
Licensure: License and current registration to practice as a Registered Professional Nurse in the state of NY required.

Education: Bachelors Degree in Nursing required; Masters Degree in Nursing preferred.

Experience: Minimum of five years of nursing experience required of which two or more years have been in a clinical specialty such as blood pressure control or cardiac rehabilitation including medication management required. Experience in providing patient counseling on behavior change required, preferably in blood pressure control or cardiac rehabilitation. Prior experience in providing education/clinical care that is culturally sensitive to the targeted study population. Personal computers skills required, including Microsoft Office Suite. Prior experience using the Pen Tablet to gather clinical and patient information preferred. Exceptional follow up and follow through skills required. Excellent communication and interpersonal skills with a strong telephone presence also required.

Preferred candidates would also have a research background with some experience working in home care community.
