Summary Statement:The Intake Coordinator will be responsible for the intake of new referrals and the coordination of client care staff under the direction of a Registered Nurse. The coordinator will also be responsible for providing assistance to administrative/ supervisory staff as needed. Reports To: Director of Clinical ServicesSupervises: N/A Education and Experience:
* Currently licensed in the state of Ohio* Has at least one year of experience as a LPN/LVN and shall work under the supervision of a registered nurse* Excellent computer skills* Intake experience preferredSpecific
* Knowledgeable of nursing and medical practices, procedures, regulations and guidelines that pertain to home health care* Excellent customer service skills* Ability to interact with all levels of personnel* Effective organizational skills* Read, write, and communicate in English objects and clients* Ability to function under the direction of a Registered Nurse
