RESPONSIBILITIES AND QUALIFICATIONS:
Manages the day-to-day operations of an Interdisciplinary Care Team. As part of the nursing leadership team, supports the VNSNY mission, vision, values and strategic initiatives to ensure the highest level of patient-centered care, including quality and financial outcomes. Works under general supervision.
Licensure: License and current registration to practice as a Registered Professional Nurse in New York State required.
Education: Bachelor&rsquot;s or Master&rsquot;s degree in Nursing required. Master&rsquot;s or Doctoral degree in a healthcare related field preferred.
Certification: Certification in a clinical field or Nursing Administration is preferred.
Experience: Peds experince required. Minimum three years of clinical experience in home care, care management, hospital or related setting required. Minimum of two years progressive experience in clinical management preferred. Demonstrated knowledge of the regulatory and reimbursement requirements governing Home Care preferred. Effective oral, written communication and interpersonal skills required. Experience with managing budgets, analyzing data, and quality improvement preferred.
