Physician Internal Medicine Geriatrics

Portland, OR Providence Medical Group is recruiting for a BEBC Internist or Family Medicine physician with a fellowship or advanced training in Geriatrics to join our provider network in the City of Roses. See geriatric patients as their primary care physician in our innovative practice model. We are developing a virtual clinic of geographically dispersed PMG providers who will offer coordinated care to all PMG clinic patients who are residing in an SNF or ICF. The overall goals of our new model are to improve the quality outcomes for the patient; ease communications and care coordination for primary care providers with the facilities; improve communications with families; and reduce unnecessary hospital admissions. Fellowship in Geriatric Medicine preferred but not required. Our 550-plus physicians have a wide range of clinical expertise, including Family and Internal Medicine, Womens Health, Dermatology, General Surgery, Neurology and Hospital Medicine.
