Description


- Coordinates with the RN to provide skilled nursing care as approved by the patient's physician, demonstrating knowledge and compliance with all applicable Agency policies and procedures, state and federal guidelines, CHAPS standards, and parameters of licensure. Follows care pathway as outlined when providing care to terminally ill patients as established by the RN; clarifies specifics prior to visit. Assists in identifying patient/family education and care needs in order to achieve optimal patient outcomes. Identifies current and potential problems, barriers and works toward appropriate resolution. Weight: 35%

- Reports identified problems to the physician in a timely manner. Accepts orders and notifies the patient's case manager and supervisor as appropriate. Communicates changes in patient status, missed / moved visit activity, and schedule changes. Weight: 15%

- Completes and submits paperwork in a timely manneraccording to Agency requirements. Participates in clinical conferences, educational presentations, and performance improvement activities of the Agency. Participates in the Call Rotation and the Emergency/Disaster Plan as needed. Weight: 17%

- Performs other duties as assigned.

Requirements

A. EDUCATION

Graduate of an Accredited LVN School in Texas.

B. EXPERIENCE

Minimum of one (1) year of clinical experience as a LVN in acute care setting. Hospice experience preferred.

C. LICENSURE / CERTIFICATION:

Current LVN license in Texas. CPR certification. Must have a current Texas driver's license with current automobile insurance.

D. EQUIPMENT / SKILLS:

Personal protective equipment, blood pressure equipment, needles, syringes, HME in patient home, telephone system, pager, written and verbal communication skills, knowledge of medical terminology and basic disease processes. Employment requires the use of a pager.

E. WORKING CONDITIONS / PHYSICAL REQUIREMENTS:

Work involves continuous exposure to pleasant and unpleasant working conditions or undesirable elements. May involve exposure to a variety of components that are potentially hazardous or harmful and requires exercise as well defined precautions for protection from injury or illness. May occasionally involve exposure to harsh weather and hazardous driving conditions.
