The Care Manager will work at Administrative South Building (ASB) in Tukwila, planning the discharges and follow up care for Group Health patients hospitalized telephonically in non-rounded hospitals. Some weekends and holidays are required, and scheduled days of the week are variable. This position is primarily a telephonic position, however, some travel is involved to meet with clinical care teams for patient and family conferences.
Primary responsibility is to focus on achievement of optimal patient health care outcomes while ensuring appropriate utilization of health care resources. Working closely with Group Health teams and network providers, the Liaison Nurse will establish a collaborative plan of care to assure adherence to the medical plan, improvement in functional status, and improved ability to self-manage. Serves as the liaison across the internal GHC care continuum and between GHC and all externally contracted providers, facilities, and resources and provides feedback to the organization regarding the service and quality of contracted services. The Liaison Nurse collects data and provides input to leadership regarding issues or concerns related to utilization, cost, quality, service and care delivery to patients.
This is a part-time position working Friday, Saturday and every other Thursday with hours of 8:00 am - 4:30 pm
Minimum Education Requirements
Bachelors Degree BA or BS
Bachelors Degree BS in Nursing
Minimum Knowledge/Skills Requirements
Effective, independent nursing judgment and skills, and use of evidence based clinical decision making criteria.
Advanced Knowledge in management of chronic disease process, nursing process and collaborative care planning. Demonstrated skill and experience in effectively collaborating with care team members.
Telephonic Case Management, Utilization Review
Minimum Experience Requirements
3 years Recent RN medical/surgical/ambulatory clinical experience required.
Health Plan experience, Telephonic Case Management, Utilization Review.
2 years RN experience in ambulatory case management, care coordination or disease management.
Minimum License/Certifications Requirements
At time of hire Current Washington State RN license
Able to obtain Certification in case management field within two years of hire
If no current CPR must be within 90 days of hire
Certified Case Manager (CCM)
