Position Summary

- Completes patient/family evaluation to determine psychosocial needs, training/education needs, barriers to discharge, discharge environment, resources needs and opportunities needed for a successful Rehabilitation stay and discharge. Communicates assessment results to the team, payer and other stakeholders as appropriate. Maintains and updates discharge planning and details on an ongoing basis.
- Coordinates case management process from patient's entry into the Rehabilitation system to post-discharge, including outpatient settings. Coordinates care and resources with physicians, Rehab team members and other stakeholders to achieve optimal patient outcomes. Follows Rehab patients to other acute units when Rehab stay is interrupted.
- Establishes daily and weekly communication with all assigned inpatients and their families or designated representatives, to foster communication, satisfaction, problem resolution and discharge readiness.
- Serves as the primary communication conduit between the patient/family, the Rehabilitation team and payer to ensure knowledge of and consensus regarding treatment goals, discharge plans, timelines and expected outcomes.
- Identifies and provides outreach in the form of information and referrals for community resources to patients and families such as shelter, transportation, future treatment plans and financial assistance. Works with patient and family members to identify the most appropriate resources and makes arrangements for the services.
- Serves as the team leader to ensure accountability for discharge planning and implementation, ensures timely follow-through for discharge arrangements.
- Assesses and discusses funding and insurance issues with client, family, and healthcare providers to enhance cost effective utilization of services and quality outcomes.
- Negotiates with third party payers relative to benefit levels, eligibility, utilization review, and reimbursement.
- Identifies actual and potential delays in service requests or treatment and communicates them to Rehab team so steps can be taken to eliminate or minimize delays.
- Works with the Rehab team to facilitate high-quality, cost effective outcomes through discharge preparation in accordance with patient/family needs, payer considerations as well as national and regional length of stay standards and community norms.
- Facilitates patient/family access to resources to enhance the rehab experience, achieve outcomes and implement effective discharges to the highest level of independence.

This position provides professional counseling services of a preventative and developmental nature through direct consultation with patients. Therapy activities include individual, group and couples counseling. The incumbent develops program criteria for area of specialization such as couples or addictive behavior counseling. This position may be responsible for supervising the work of Social Workers, Interns or other caseworkers.

Works in close collaboration with all members of the treatment team including medical doctors, to further their understanding of significant social and emotional factors contributing to the patient?s well being.
