REHAB JOB SUMMARY:
The home health rehab therapist provides direct patient care in the patient s home environment as part of a multidisciplinary team including nursing, PT, OT, speech, social work and home health techs. The therapist will evaluate patients and use professional judgment to design the individualized plan of care which best meets the patient s needs. The therapist will provide direct, one-on-one patient care, and will also supervise care provided by assistants. The home health agency relies heavily on technology; email is a primary means of communication and the therapist will use a laptop to complete documentation in an electronic record. The patient population varies widely across the spectrums of acuity, age, and diagnosis. The therapist works under the direction of a clinical team manager with support from a rehab clinical coordinator. Employment begins with a 4-6 week orientation with a seasoned preceptor to ensure that the clinician is well prepared when they begin treating on their own. Additional job duties include performance improvement opportunities, policy and procedure review and development, research, etc. Working in home health provides endless variety of setting, scenery, and clinical challenges, and provides a unique opportunity to enter and work within the real life environments of our patients, bringing meaningful change to their daily lives.

QUALIFICATIONS:
Associate Degree as Physical Therapy Assistant.
Virginia or West Virginia licensed or eligible (as applicable to state practicing in).
Basic computer and keyboarding skills or participation in computer training within first year of employment.
Competent to successfully interact with patients according to their special needs which may apply based on the age of the patient served.
Must not have been convicted of or have pending charges of any barrier crime.
Current driver s license and reliable transportation.
Must have access to transfer electronic data.
