Provide nursing services to patients and families within the scope of practice of the LPN license as defined by the State of New Hampshire Board of Nursing. Efficiently gathers information from patients and/or caregivers in person or on the phone and provides telephone nursing care. Resources available for telephone care include departmental guidelines, DHK standard telephone triage books, and provider specific information. The LPN recognizes their limits of knowledge and experience and plans for management of situations beyond their expertise, using provider and RN available resources in the department. Develops and maintains the individual instruction sheets related to the individual provider practice preferences to utilize for position coverage in his/her absence. Must maintain current State of New Hampshire LPN nursing Licensure and BLS certification. Must possess excellent communication, interpersonal and organizational skills. Ability to relate well at all levels. Full-time 40 hours/week Days
