Summary:
Provides direct care to Dialysis patients under the supervision of a Registered Nurse.
Job Duties:
Assists in the initial and ongoing patient assessment through data collection.
Completes pre and post dialysis patient data collection: Vital signs, weight, review recent lab data, performs physical assessment including lung sounds, neck vein distention, and observation for peripheral edema.
Plans patient care based on the Nursing Assessment and includes patient and significant others as well as relevant principles of biological, physical and social sciences in the plan of care.
Demonstrates knowledge of medications and their administration based on the age of the patient and clinical condition according to approved standards and protocols.
Instructs patients and family members and documents teaching done in progress notes and on the patient teaching sheet.
Qualifications:
Currently licensed LPN in the Commonwealth of Massachusetts
Graduate of an accredited school of practical nursing
Dialysis experience preferred
CPR certified
Part time Day & evening shift Licensed Practical Nurse Experience required
