Job Description

Southeast Georgia Health System's Senior Care Center has an opportunity for a part-time Licensed Practical Nurse (LPN) to join our long-term care team!
The Licensed Practical Nurse (LPN), Long-Term Care:
* Under the supervision of a Registered Nurse, provides patient care using the nursing process of assessment, planning, implementing, and evaluation
* Uses good judgment
* Utilizes patient care supplies/equipment in a fiscally responsible manner
* Exhibits positive interpersonal skills
The LPN, Long-Term Care performs the following essential responsibilities:
* Effectively manages the care of assigned patients using knowledge, clinical skills, judgment and decision making skills for the specific area, under the direction of a Registered Nurse
* Delivers patient care utilizing guidelines of care and appropriate resources, including personnel, services, supplies and equipment
* Reviews, evaluates and takes appropriate action regarding results of diagnostic tests
* Administers medications according to SGHS policy
* Assists the RN in performing the nursing assessment to identify and prioritize physical psychosocial and cultural needs in order to develop a plan of care
* Evaluates and reassesses nursing interventions based on interdisciplinary plan of care and makes revisions as appropriate
* Assists with the development and implementation of a patient/family education plan
* Documentation meets compliance with regulatory standards

The ideal LPN, Long-Term Care candidate should possess the following minimum qualifications:
* Current Georgia LPN license
* Nursing skills as defined in the law governing the practice of nursing in Georgia
* Current American Heart Association CPR Certification (BCLS) preferred
* Long-term care experience preferred
