JOB DESCRIPTIONAPPLY SHORT DESCRIPTION:
Provides case management, care coordination and treatment adherence services for 50 HIV positive individuals or persons at high risk for HIV infection.
Provides emergent case management services for EIP patients as needed.
Complete case management or PrEP assessments as applicable.
Develop client service care plans.
Monitor and evaluate service care plans quarterly.
Assess all EIP clients requesting or referred for CM or PrEP services and identify immediate/urgent needs and take appropriate action.
Will meet objectives for the provision of referrals and linkages to support services.
HIV Counseling and Testing as needed.
Services provided in accordance with AACO and/or the NJDHSS standards.
Bachelors Degree or licensed health care professional (LPN or RN)
1 year or more experience in the field of health care, case management or Social Services.
Experience in the field of HIV preferred.
Extensive knowledge of available financial and social service resources in southern New Jersey.
Excellent interpersonal skills.
Required License/Certifications/ Registration:
AACO or NJ HIV Case Management Certification preferred.
NJDHSTS PrEP counseling course (will train). HIV Counseling and testing (will train).
