RESPONSIBILITIES AND QUALIFICATIONS:
* Oversees all components of assignment and schedulingof weekday and weekend patient visits across all disciplines.
* Provides support to clinical operations and facilitates communication between business support, Quality Management Services and CHHA Operations.
* Communicates and coordinates the care of patients as needed. Serves as a support for all field staff in addressing issues and building clinical capacity.
Licensure: License and current registration to practice as a Registered Nurse, Physical Therapist, Occupational Therapist or Speech Language Pathologist in New York State required.
Education: Bachelors Degree in Nursing, Physical Therapy, Occupational Therapy, or a Masters Degree in Speech Language Pathology required. Masters or Doctoral Degree in Nursing, Physical Therapy or Occupational Therapy preferred.
Experience: Minimum of five years experience as a Nurse, Physical Therapist, Occupational Therapist or Speech Language Pathologist required. Knowledge of Medicare, Medicaid and third party payers required. Team leadership and facilitation skills required. Knowledge of Federal/State regulations that govern Home Care preferred. Effective oral/written communication and interpersonal skills required.
