The MTM Pharmacist, with advanced training and credentials, will provide clinical services to patients with chronic kidney disease. The MTM Pharmacist will also play integral role in transitions of care working directly with our healthcare teams.
The overall goal of theMTM Pharmacist is to work collaboratively as a member of the DCI team to optimize patient care and improve outcomes.
**Essential Duties and Responsibilities:**
+ Activities will include medication reconciliation, patient education, acute and chronic disease/therapy assessments, and developing and implementing pharmaceutical care plans.
+ Plays an integral role in transitions of care, working directly with the healthcare teams.
+ Maintain a patient case load and provide person-to-person telephonic and videoconference based consultations for patients enrolled in MTM program.
+ Will be responsible for assisting in continuing education and training of MTM pharmacists and employees as it relates to current treatment guidelines and drug information.
+ Evaluation of each chronic dialysis patient enrolled in MTM program to include patient interview, chart review, and/or communication with other healthcare disciplines about the health status of the patient
+ Identify and plan for resolution of medication-related problems
+ Evaluate appropriateness, effectiveness, safety and adherence of drug therapy
+ Documentation of medication therapy management services within designated software application
+ Provision of medication therapy management summary reports to patients and providers as appropriate
+ Recommendations formulated and communicated to nephrologist in a manner conducive to both parties involved
+ Communication with Primary Care Provider(s) and insurance company representatives regarding treatment decisions
+ Provide drug information and education to staff and patients as requested
+ Provide continuity of care to the patient load assigned
+ Generate CQI report of progress at each clinic
+ Develop and maintains good relationships with all DCI facilities for which services are provided
+ Create and implement medication error reduction process that identifies and addresses real or potential medication error(s) to include prevention and reporting
**Certificates, License and Registration:**
+ Required: Licensed Pharmacist in good standing.
+ Highly Desired: Board Certified Ambulatory Care Pharmacist (BCACP) and/or equivalent, Board Certification (BCPS), Certified Disease Manager
**Education and/or Experience:**
+ Graduation from accredited school of pharmacy.
+ Pharm D, Registered Pharmacist
+ Highly Desired: Residency and/or Fellowship.
+ Completion of training in nephrology or internal medicine preferred.
+ Licensed to practice pharmacy in several states.
+ Observes strict confidentiality of patient and personnel information.
+ Conducts self in a professional manner as demonstrated by appearance and attitude, by maintaining a clean and orderly environment, and by dedicating work time to meet patient and DCI needs.
+ Recognizes and responds appropriately to emergency situations.
+ Understands clinic agreements and contracts.
+ Follows the facility attendance policy (absence and tardy).
+ Displays a positive attitude toward patients, co-workers, physicians and DCI.
+ Assists in meeting clinic goals.
+ Performs other duties as assigned.
