RESPONSIBILITIES AND QUALIFICATIONS:
Provides in home treatment for depression to clients in PEARLS program, utilizing PEARLS model of care. Provides education, encouragement and support to individuals who screen positive; schedules individuals for in home treatment. Coordinates effective communication between the team members to ensure appropriate referral and linkage to the full array of necessary physical and behavioral health services to guarantee the patient receives the ultimate benefit. Works under general direction.
Licensure: Licensed Social Worker (LSW), Licensed Mental Health Counselor (LMHC), Licensed Marriage Family Therapist (LMFT) or Licensed Psychologist required. NY state driver's license required; must be able to drive agency vehicles.
Education: Master's degree or higher in Psychology, Social Work, or related field required.
Experience: Minimum of one year of professional experience in providing mental health treatment required. Experience working with older adults required. Effective oral/written/interpersonal communication skills required. Experience with CBT models of treatment preferred. Computer fluency preferred. Experience providing field based services preferred. Bilingual: Cantonese and/or Mandarin required.
