Responsible for coordinating the care of patients and their families or significant others within the emergency room (ED) setting.
Monitors admission status, medical appropriateness, and coordinates discharge planning for all patient populations including behavioral health, extended care, and other appropriate patient placement as needed. Monitors and evaluates patient re-admissions.
Other duties as assigned.
Bachelor's degree in Nursing preferred.
A minimum of two years of clinical practice in designated specialty is preferred. Prior utilization management or case management experience is preferred.
A minimum of five years of acute care nursing experience is required.
Analytical skills necessary to assess patient's discharge needs and develop solutions to problems concerning patients' mental status or physical well-being to ensure optimal patient outcomes. Interpersonal skills necessary to instruct and maintain effective contacts with a variety of hospital personnel.
Professional knowledge of nursing theory and practice at a level normally acquired through completion of a minimum of two years education at an accredited School of Nursing in order to be eligible for licensure as a Registered Nurse is required.
Requires current state of Wisconsin Registered NurseLicense or a Multi-state Nursing License from a participating state in the NLC (NurseLicensure Compact). American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required within 90 days.
