Facility: Hospice - Meadville at Meadville
Department: Hospice - Administration
POSITION SUMMARY: The Nurse Liaison is responsible for admissions and all hospital-based referrals and hospice discharges. This individual will round in the hospital daily and complete home admission if time available. MINIMUM QUALIFICATIONS: Registered Nurse in State of PA, 3-5 years Hospice experience. Previous customer service experience, excellent communication and organizational skills, Strong time management skills and sales experience preferred. Full-time Day shift
