JOB DESCRIPTIONAPPLY Full time position available in our Internal Medicine Department located in our Framingham site. Candidate must speak Portuguese.
Under the general supervision of the supervising physician, coordinates comprehensive health care needs of adult patients in an internal medicine practice. Provides direct care through assessment, diagnosis, and treatment of acute minor and chronic conditions within the scope of the Advanced Practice Nurse as described by the American Nurses Association (ANA). Coordinates and monitors patients care throughout the continuum of healthcare services. Participates in the education and research activities of the group.
Principal Duties and Responsibilities:
Collaborates with the supervising physician and healthcare team to provide comprehensive, patient-centered care to ambulatory patients with acute or chronic disease. Participates in daily patient care huddle with the healthcare team to prioritize and meet or exceed patient needs.
Responsible for the diagnosis and management with acute, episodic or chronic illness within the scope of clinical policy and that of Advance Nurse Practice as outlined by ANA.
Obtains and records medical histories from patients using motivational interviewing techniques.
Performs and records physical examinations and assessments of patients in the ambulatory setting.
Plans and initiates health maintenance and disease prevention for patients, including education, instruction and counseling. Coaches patients regarding care, treatment, and behavior change as needed. Tailors communication style to meet the special needs of the patient.
Documents necessary information appropriately in the medical record within 24 hours after patient interaction.
Initiates and coordinates appropriate referral of patients to specialist and ancillary services.
Orders appropriate laboratory, diagnostic tests and consults according to clinical guidelines and established Charles River Medical Associates guidelines.
Provides oversight for healthcare monitoring and population management to include appropriate monitoring and follow up on referrals to specialists and ancillary services. Works with care management team to ensure vulnerable populations are monitored and provided coaching.
Reviews findings and treatment plan with physician preceptor as deemed necessary by Nurse Practitioner based on complexity of care to be delivered, the individual, Nurse Practitioners knowledge base pertaining to that care, scope of Advanced Nurse Practice and/or agreed upon by the Nurse Practitioner/Physician team.
Triages telephone calls of patients.
Complies with all standards related to patient confidentiality/HIPAA.
Ensures that the practice code of conduct and all compliance and ethical standards are maintained.
The Nurse Practitioner, with legal prescriptive authority, may prescribe medications in accordance with an agreement defined by the supervising physician and the Nurse Practitioner. Medications are prescribed within the boundaries of accepted practice. If a situation is beyond these limits, the Nurse Practitioner will seek consultation with the collaborating physician. These prescribing guidelines will be reviewed by the physician and Nurse Practitioner every 3-6 months.
The Nurse Practitioner will participate in professional meetings, committees, seminars, and other continuing education to keep abreast of changes in professional practice to enhance professional growth, and to meet professional standards.
The Nurse Practitioner provides support to nursing staff, and may act as a resource person for nursing.
Participates in quality assurance review and initiatives.
Participates in research within Charles River Medical Associates.
Participates in the teaching and precepting of Nurse Practitioner and medical students
Massachusetts licensed Registered Nurse
Authorized to practice in the expanded role of a Nurse Practitioner by the Board of Registration in Nursing for the Commonwealth of Massachusetts
Masters degree in Nursing
Certification as an Adult Nurse Practitioner or Family Nurse Practitioner
Demonstrated competency in clinical practice.
Must speak portuguese
