Nurse Practitioner

Nurse Practitioner Facility: Saint Luke's Home Care and Hospice Department: HHC Administration Schedule: Full Time Shift: Day Hours: 0800 1700 Job Details: * Master's Degree * RN Required * 1 year of experience required * Under the direction of the Referral Services manager, the Nurse Practitioner/Program Coordinator will be responsible for the development of a Chronic Disease Management Program, to include transitional care, telehealth and disease specific pathways in order to best manage the chronically ill patient. The Nurse Practitioner/Program Coordinator will work to build relationships with physicians, hospitals, and other facilities in order to form partnerships in the management of patients with chronic diseases. The Nurse Practitioner/Program Coordinator will work collaboratively with the clinical practice councils to develop disease specific pathways for home health. The Nurse Practitioner/Program Coordinator is responsible for ensuring that all activities are relevant, accurate, and targeted to help Home Care and Hospice meet its mission, strategic goals and objectives.
