Department: Managed Care

PURPOSE OF POSITION:
Provide hospital-based (but not hospital care limited) patient care to Managed Care Patients.

QUALIFICATION REQUIREMENTS:
To perform this job successfully, an individual must be able to meet expectations relative to the purpose of the position and perform each primary responsibility satisfactorily. The requirements listed below are representative of the knowledge, skill, education, experience, and/or ability to perform this position successfully.


Current MA License, MA CSR, and Federal DEA Certificates
Five or more years of clinical experience preferred
Experience in a managed care environment preferred but not required
Comfortable evaluating and managing non-surgical diseases of adults in the following settings:

Inpatient
Outpatient
Skilled nursing facility
In partnership with home health care agencies

Understands quality in medical care
Skilled and comfortable leading, teaching, learning from and working with nurse practitioners
Excellent communication and negotiation skills

EDUCATION:
Nurse Practitioner shall be trained in Internal Medicine or Family Practice and will maintain all required certifications and qualifications. Additional training or certification in Geriatric medicine is a plus, but not required.

PRIMARY RESPONSIBILITIES:
The position is a hospital based (but not hospital care limited) nurse practitioner role to care for managed care patients. The practitioner will, in partnership with the Continuity Care Physician:


Evaluate managed care patients who come to the emergency department of Signature Healthcare Brockton Hospital.
Triage those patients appropriately to home, home with assistance, inpatient, observation, SNF or LTAC care.
Round daily (generally multiple times) on managed care inpatients in conjunction with the Continuity Care Physician, providing appropriate evaluation and management strategies.
Coordinate with the Full Time 7 days on / 7 days off
