JOB DESCRIPTION

The NP/PA Extensivist is a licensed Nurse Practitioner or Physician Assistant who provides services to the senior patient population across multiple settings. S/he will see, treat and manage patients primarily in the hospitals and skilled nursing facilities with scheduled clinic responsibilities. S/he will see post-discharge patients in the skilled nursing, intermediate care and assisted living facilities, as well as in the home.

Secondary duties may include new member evaluations and chronic disease management follow-up in the in the skilled nursing, intermediate care and assisted living facilities, as well as in the home.

GENERAL DUTIES/RESPONSIBILITIES:

Evaluates patients in the hospital, communicates with member, family and care team to coordinate care plan. Communicates with Regional Medical Officer (RMO) to assess appropriateness of plan and unmet needs.

Evaluates post-discharge patients in skilled nursing, intermediate care and assisted living facilities, as well as in the home.
*

Participates in daily 1:1 rounds with RMO to discuss active medical issues and plans for all hospital and SNF patients.
*

Participates in daily multi-disciplinary Alignment team rounds to discuss active issues, discharge planning and follow-up for all hospital and SNF patients.
*

Coordinate care with hospital and Alignment care managers to ensure smooth transitions from the hospital to post-acute care settings.
*

Across all settings of care and with assistance of RMO, create a comprehensive and coordinated plan of care which takes into account information from medical records, the patient and their families, primary care physicians, and specialists as necessary.
*

Maintain appropriate and timely documentation of all clinical encounters. Inpatient and SNF encounters to be completed within 24 hours, outpatient encounters within 48 hours.

Participate in telehealth monitoring rotation.
*

Participate in on-call schedule as needed. (May vary by location.)
*

Participate in quality improvement projects as necessary.
*

Participate in weekly operations, regional and local staff meetings (approximately one hour per week)
*

Participate in monthly educational forums (one hour per month)

MINIMUM REQUIREMENTS:

*

Minimum Experience: Prefer prior inpatient experience in general medicine or medical sub-specialty.
*

Significant outpatient general medicine experience will be considered in lieu of prior inpatient experience.
*

Education: ARNP or PA-C.
*

Licensure: Active license in the state in which you are applying.
*

Other: Interest in healthcare improvement, geriatrics and managed care preferred.
*

Strong communication skills and ability to work collaboratively essential.
*

Travelrequired within Wake County
