Job Description

Summary: The Adult Cardiac Advanced Practice Nurse is a registered professional nurse who uses specialized skills in the care of patients and families undergoing cardiac operative and other invasive procedures. Completes comprehensive patient health evaluations in collaboration and consultation with Cardio-thoracic Surgery and Cardiology. Assesses and coordinates the educational and psychosocial needs of the patient and family. Reviews plan of care with bedside nurse and charge nurse. Coordinates discharge planning in collaboration with Care Management. Essential Duties and Responsibilities: include the following. Other duties may be assigned. 1.0 Manages comprehensive and individualized care for patients and families in the Inpatient phase of hospitalization. 1.1 Completes preoperative andor pre-procedure evaluations. 1.2 Completes integrated assessments. 1.3 With the collaboration of Cardio-thoracic Surgery andor Cardiology or by following established protocol, orders additional tests or consults to complete the preoperative andor pre-procedure workup. 1.4 Performs, interprets and communicates results of common diagnostic tests. 1.5 Consults and communicates with Cardio-thoracic Surgery andor Cardiology regarding abnormal findings in the evaluations of patients. 2.0 Manages comprehensive and individualized care through the Inpatient and immediate post-discharge phase of the patients hospitalization experience. 2.1 Develops a comprehensive individualized plan of care for patients that includes teaching, discharge planning and necessary referral to other members of the health care team, with specific emphasis on anticoagulation therapy management. 2.2 Demonstrates expertise in caring for patients across the age continuum. 2.3 Participates in physician rounds, as applicable, with emphasis on patients transferring between cardio-thoracic and Cardiology Services. 2.4 Identifies unstable or complex patient care situations and facilitates appropriate intervention with appropriate resources. 2.5 Facilitates patient discharge by coordination of appropriate services. 2.6 Demonstrates critical thinking and diagnostic reasoning skills in clinical decision making by the identification, evaluation and management of the individual needs of patientsfamilies. 2.7 Demonstrates the ability to utilize basic equipment in a safe manner. 2.8 Recognizes emergency situations and initiates effective emergency care. 2.9 Bases decisions madeactions taken on relevant scientific principles, established standards of care andor research findings. 3.0 Provides education to nursing staff, hospital staff, patients and families while utilizing teachinglearning theory and appropriate teaching strategies. 3.1 Promotes an environment that facilitates learning by responding to appropriate patientfamily preferences, priorities and readiness to learn. 3.2 Evaluates the effectiveness of patientfamily teaching and completes all documentation in the patient chart. 3.3 Updates own knowledge base throughout the year. 3.4 Completes all identified clinical competencies. 3.5 Serves as a resource person in CTUCCU,10 East, and 10 W 3.6 Provides educational programs to professional and paraprofessional personnel in area of expertise. 3.7 Mentors nursing personnel in all levels of clinical practice. 4.0 Communication. 4.1 Communicates effectively with patientsfamilies, physicians and ancillary services at all levels to assure coordinated efficient patient care. 4.2 Documents the plan of care, interventions and evaluations in a clear and concise manner. 5.0 Leadership. 5.1 Provides leadership in the development and implementation of changes in nursing practice that positively impact patient outcomes. 5.2 Collaborates with other leaders and staff to effectively achieve needed change. 5.3 Workscollaboratively with medical and nursing staff to facilitate creative problem-solving and provide professional support. 5.4 Participates and initiate
