Job Summary: Assists patients with activities of daily living, provides basic nursing care to non-acutely ill patients, and assists in maintenance of a safe and clean environment under the direction and supervision of the Registered Nurse in charge of the unit and/or district. Requirements: Participates in patient assignment conferences with the Registered Nurse in charge of the unit and/or district and fully understands patient assignment. Takes and records temperature, pulse, respiration, blood pressure, height, weight, I&O, and other measurements as applicable, in an accurate and precise manner. Records all vital signs immediately after taking, in the log book and reports all variations from normal to the Registered Nurse in charge of the unit and/or district. Demonstrates competence and an awareness and understanding of patients needs for comfort and safety, in providing morning, evening and general care routines in accordance with established Nursing Care Procedures and the hospital s Administrative Policies and Procedures. Prepares patients for meals, sets up each patient and their tray, assists in feeding patients (if necessary), positions patients to avoid choking and ease in eating: regularly provides patients with fresh water (at least once a shift) and other nourishments according to patient s needs. Utilizing knowledge of techniques and regulations appropriate to the ages of patients served. Communicates clearly and tactfully with patients. Demonstrates competence in performing treatments and procedures such as enemas, applications of heat and cold compresses, sit bath, skin preps and EKG s. Records and/or reports to the Registered nurse in charge of the unit and/or district, any treatments, results, observations, patient complaints, and other pertinent information regarding the condition of the patient Demonstrates a reliable and dependable ability to follow through with designated responsibilities in the collecting, labeling, and transporting of specimens to the proper area at the appropriate time: always correctly labels type of specimen collected, including patients name and room number. Completes Patient Belonging Form on admission and instructs patient in regards to correct storage of patients valuables, eye glasses and dentures; use of equipment, bed controls nurse/patient communication systems; and location of bathroom and emergency call signal. Communicates clearly and tactfully with patients. Answers patient call lights (whether that of assigned patient or not), anticipates patient needs and checks on patients regularly; demonstrates a cooperative attitude when assisting patients. Communicates clearly and tactfully with patients. Completes accurate documentation on all appropriate log books by the end of the shift; includes signature and title; when appropriate, maintains proper charting using only procedural abbreviation/short codes as outlined in the procedural manual. Demonstrates a knowledge of Infection Control and Isolation Procedures; strictly adheres to policies and procedures as outlined in the departmental manuals; washes hands before and after each patient contact. Demonstrates a knowledge of correct bed making procedures (occupied, unoccupied, post-up) as outlined in the procedure manual. Recognizes abnormal vital signs and reports them to the Registered Nurse in charge of the unit and/or district immediately: always reports any changes in patient condition to appropriate nursing personnel. Observes patients and reports problems (bleeding, drainage voiding, machine malfunctions, IV drip, safety situations, etc.) immediately and/or current status to the Registered Nurse in charge of the unit and/or district. Ensures delivery of excellent customer service resulting in positive and improving patient satisfaction scores for department and Medical Center. Assists and accompanies patient in the admission, transfer, and discharge process; in appropriate log book documents patient admission and discharge data. Demonstrates ability to appropriately set up traction, as required. Assists with or transports patients according to proper techniques of body mechanics to protect patient and personal safety; always ensures patients safety by maintaining beds in the low position with side rails in place and locked at all times, and by properly securing patients when transporting by wheelchair or stretcher. Assists staff Registered Nurse and/or Licensed Practical Nurse with treatment procedures, as directed in an accurate and precise manner. Provides post-mortem care and transport of body in a safe, precise manner to the morgue. Attends all JCAHO required in services yearly. Regularly updates knowledge of techniques and regulations appropriate to the ages of patients served. Demonstrates a good rapport and cooperative working relationship with all members of the health care team; responds to co-workers with concern and cooperation; readily assists co-workers without being asked. Greets patients in a courteous manner, responds to patients empathy and positive interpersonal skills; introduces self and refers to patient by name. Demonstrates proficiency in utilizing computer system within department. Performs other duties as assigned. Works indoors in well ventilated rooms. Possibility of cuts or minor burns from instruments and equipment. Some direct exposure to contagion and fumes from chemicals, but risk is minimal when proper safety precautions are observed. Possibility of strains due to moving patients, equipment and instrumentation. Constant standing, walking, stooping, bending, stretching, lifting, pulling, pushing, turning and occasional sitting. Frequently lifts objects from 10 - 150 lbs. Frequently carries objects weighing up to 50 lbs. Frequently lifts with assistance, objects greater than 150 lbs. Pushes objects up to 200 lbs. Manage time appropriately to meet patient and department needs. Knowledge of care and handling of instrumentation realizing that improper care and use are extremely costly. Qualifications: High school graduate or equivalent, required. Successful completion of a Nursing Assistant/Technician Training Program Certification with American Heart Healthcare Provider BCLS One year of hospital Nursing Assistant/Technician experience preferred.
