STATEMENT OF RESPONSIBILITY:
The Nursing Supervisor is a licensed nurse who supervises Direct Care Workers services in the home setting. This is in accordance with the policies and procedures of Black Stone healthcare, Ohio State Nurse Practice Act, and regulatory entities.

QUALIFICATIONS & SKILLS:

* Currently licensed by the Ohio State Board of Nursing* Has at least one year experience as a licensed nurse and shall work under the supervision of a Registered nurse* Able to function under the direction of a Registered Nurse* Able to plan, organize, develop, implement and interpret programs, goals, objectives, policies, and procedures, necessary in the provision of care.* Able to lift 50-100 pounds* CPR desirable


RESPONSIBILITIES & DUTIES:

May include but are not limited to the following:* Collaborates with CM/Director of Nursing/CSM/family to assure all need are addressed appropriately or referred to care providers to assure quality of care is being met* Understands specific rules and guidelines pertaining to a timely completion of Start of Care, and Supervisory Visits, set up by funding sources to ensure satisfaction, safety and quality of service* Develops the Care Plan according to client's needs and payer requests, supervises the HCA to assure the tasks on the Care Plan are being provided in the scheduled time frame required.* Reviews the SR's for client signatures on every visit and documents accordingly. Notify staff and CM of any issues.* Works with and respects persons of various cultures, social economic, ethnic, religious and racial backgrounds* Works through multiple types of customer situations, find solutions to client concerns with focus on quality care and safety* Understands hands on experience of different ailments* Identifies imminent needs in a home setting to effectively address health needs in the ill, elderly or disabled* Uses caution when traveling from home to home and work in a variety of living and weather conditions * Maintains a high standard of leadership for those you represent* Possesses good nursing observation skills, and properly record/report those findings to DON and those involved in care , of any issues to ensure continuity of care with appropriate referrals to Home Health Care/ certified and House Calls by Black Stone * Builds rapport to provide premiere service levels in providing emotional and physical support to clients as needed* Participates in education programs, in-services, webinars, care coordination and seminars as directed* Directs others in education of continuity of care, aide/family educating skills checks and Hoyer/slide board demonstrations* Maintains confidentiality of client and employees and related information as stated in the Client Bill of Rights and HIPPA* Utilizes and use universal precaution techniques as necessary; with the ability to identify and implement interventions* Assists in Helping Black Stone assure TB test are administered and results are recorded within time restrictions * Answers questions and concerns for client/families/staff in regards to care safety and health.* Performs on call duties as directed by DON * Conducts chart audits to ensure accuracy of compliance requirements* Performs other duties as assigned
