JOB DESCRIPTIONDESCRIPTION/JOB SUMMARY
FULL TIME MINIMUM QUALIFICATIONS: Must have a high school diploma or GED. Must have received prior mental health services or are currently receiving mental health services. Must pass background check prior to hire. Must possess a valid driver s license from the state in which you reside, good driving record and insurable under Center s policy. Preferred to have completed certified peer specialist training and/or be wiling to complete as a condition of employment. Must be able to pass a background check. Must have a valid driver s license. Must be insurable under LCI s automobile plan. Must be able to travel locally to clients homes, training locations and other locations as necessary.
JOB RESPONSIBILITIES: Functions as a member of a multidisciplinary team to provide peer support, advocacy, coaching, mentoring, behavior modeling, assistance, and guidance for clients assigned to the Family Intensive Treatment (FIT) Team. Promote positive and collaborative relations with FIT Team and the community. Attend available training to increase skills specific to mental health and substance abuse populations. Actively participate and contribute in staff meetings, supervision, quality reviews, and monitoring activities. Maintain positive, timely, and effective communication with patients, families, team members, external and internal resource providers, referral sources, and community providers. Maintain compliance with individual mandatory staff development plan per LCI standards (60) hours per year. Maintain client s confidentiality according to LCI s policy and HIPPA requirements. Serves as a consultant to the team to ensure client advocacy, sensitivity of team members to treatment issues, and client strengths to maintain recovery and independence. Participates as co-case manager under direct supervision of clinical staff. Immediately reports risk, suicidal/homicidal statements or behaviors to clinical staff.
