The Oaks at La Paloma is currently seeking a Patient
Care Coordinator, to ensure follow up with the patients
collaterals and work in conjunction with the clinical
team to create discharge plans. The ideal candidate will
have the ability to inspire our clients confidence in
both our staff and the FRN Model of Treatment.

is located at Foundations Recovery Network s residential
treatment facility, The Oaks at La Paloma, in Memphis,
Tennessee.
Requirements:
Essential Duties and Responsibilities
Utilizes FRN endorsed, client-centered therapeutic
interventions and rapport-building practices, including
motivational interviewing techniques, to
non-confrontationally enhance consumer insight in an
effort to develop a comprehensive discharge plan
Documents referrals, family, employer, and other
contacts (during treatment and at discharge) according
to periodicity and content requirements
Maintains awareness of client needs upon entry and
utilizes that information to develop appropriate
discharge plans; implements plans accordingly
Coordinates comprehensive discharge plan with needed
resources in the community to ensure seamless community
reintegration
Coordinates with treatment team regarding daily census
Oversees all aspects of discharge planning and
coordination
Communicates referrals as necessary
Participates in meetings with other departments to
discuss policies and concerns and to make
recommendations to improve service and ensure more
efficient operation, as required
Leads Psycho-educational groups, when assigned,
related to discharge planning
Ensures communication with employers is adequate to
secure jobs utilizing FMLA, COBRA, work releases, and
other tools to maintain employment while in treatment
Education and Experience
Bachelor s degree in social work, counseling,
psychology or related field required
CADC certification in addictions preferred
Minimum of 5-10 years of experience in social service
setting or treatment facility strongly preferred

Familiarity with a variety of models and theories of
addiction, mental illness, and related issues
Demonstrates a broad base of knowledge in psychiatric
and addiction treatment and administration
Understanding of recovery, relapse prevention, and
continuing care for dually diagnosed populations
Ability to develop and maintain professional working
relationships with all internal/external interface
points
Excellent verbal and written communication skills
required
Must have high level of interpersonal skills, as this
position continually requires demonstrated poise,
professionalism, tact, and diplomacy
Must be able to work in a fast-paced environment with
demonstrated ability to juggle multiple competing tasks
and demands
Excellent organizational skills, with the ability to
prioritize workload and meet deadlines
Ability to work independently
Understanding of diverse cultures and gender specific
issues and the ability to incorporate needs of gender
and culturally diverse groups into the organization
Understands need for and maintains appropriate
confidentiality at all times when interacting with
patients, residents, families, visitors, referral
sources, and all other contacts
Exhibits excellent customer relations skills, as
evidenced by supportive and constructive communication
with all contacts, including co-workers, patients,
residents, visitors, families, and referral sources
Understanding of FMLA, short-term disability (STD),
and other general human resources issues
Must have strong knowledge of a variety of computer
software applications in word processing and
spreadsheets (MS Word, Excel)
