Department Description

General Pediatrics at Mount Zion, provides routine health examinations, treatment for acutely ill children and consultations for difficult to diagnose problems.

Our services include immunizations, vision and hearing screenings, health education and counseling for teens and families affected by violence.

Job Summary

The Patient Care Panel Manager Licensed Vocational Nurse, under the supervision of the Administrative Nurse Manager, guided by the policies and procedures of the Ambulatory Services and the Ambulatory Services Quality Nursing Division, and in collaboration with the physician staff, has the clinical responsibility for developing and maintaining a high standard of patient care. S/he is responsible for maintaining a safe environment for patients and staff. All components of nursing care will be individualized to meet the patient s physiological and psychosocial developmental level and needs. S/he consistently projects a professional and courteous demeanor, helps to establish and maintain the highest service standards for the practice and shall comply with the Medical Center House and Telephone Standards. The Licensed Vocational Nurse utilizes the nursing process to assess and identify the individual needs of patients through observation of disease symptoms, emotional state, laboratory reports, cultural and socioeconomic background and respond to medical regimen and treatment.

The Patient Care Panel Manager LVN will play a critical role in the patient experience including optimizing clinic flow, providing in and outreach to patients and working collaboratively with the team to create a patient centered environment.

This employee will utilize computer skills in Apex and other UCSF computerized programs. S/he supports the standards of the policies and procedures of the practice, and is responsible for maintaining current knowledge of pharmacology and competency in modes of administration, and complies with all Ambulatory Services and Medical Center policies and procedures for infection control, safety, and administrative and clinical practice.

In addition, the Patient Care Panel Manager LVN will assist with delivering and maintain a high standard of patient care using the Patient Centered Medical Home model. S/he may also assist with APEX chart abstraction, scheduling, pending orders etc. S/he will actively lead, and participate in multidisciplinary quality improvement teams to improve the health of targeted primary care patients across the major UCSF Health Primary Care Practices.

The flexibility to orient and work at all UCSF Medical Center locations is required.

Required Qualifications

* One to two years of related experience in similar environment as an LVN or MA

* Active Licensed Vocational Nurse licensure in the State of California

* Active American Heart Association CPR certification

* Knowledge and abilities essential to the successful performance of the duties assigned to the position

* The flexibility to orient and work at all UCSF Medical Center Locations

Preferred Qualifications

* Experience with Epic electronic medical record

* Familiarity with the principles of a Patient Centered Medical Home

Licensure/Certification

* Active Licensed Vocational Nurse licensure in the State of California

* Active American Heart Association CPR certification
