Job Summary

The Pediatric Case Manager in collaboration with the patient/family and interdisciplinary team, facilitates and coordinates resources and services in response to individual patient s needs and fiscal realities, performs utilization review activities, and oversees coordination of complex discharge planning for assigned case load. The Pediatric Case Manager is an active member of the interdisciplinary team contributing to team goal setting. The Pediatric Case Manager acts as a consultant to the clinical team, service lines, and other departments, and participates in program development and quality improvement initiatives.

The Pediatric Case Manager provides specialized assessment for assigned service line(s). Prior to discharge, the case manager is responsible for initiating and coordinating the out patient services necessary for the multiple disciplines. The Pediatric Case Manager demonstrates the ability to work with any patient regardless of race, gender, religious affiliation, cultural beliefs, lifestyle, age, disease process and/or treatment plan.

The flexibility to orient and work at all UCSF Medical Center locations is required.

Required Qualifications

* Possesses a Bachelor's Degree in a healthcare related field with a minimum of two years of acute hospital clinical experience

* Knowledge and abilities essential to the successful performance of the duties assigned to the position

* Self directed, assertive and creative in problem solving, systems planning and patient care management in a high volume work environment

* Basic computer skills

* Ability to work effectively and collaboratively with interdisciplinary teams

* The flexibility to orient and work at all UCSF Medical Center locations is required

Preferred Qualifications

* RN licensure with a Master in Nursing or Healthcare related field

* Recent experience in pediatrics, intensive care nursery or obstetrics

* Recent experience in case management, utilization reviews, or discharge planning
