Responsible for the delivery of patient care under the supervision of the Physical Therapist Manager and in accordance with the treatment plan. In addition, the PT is responsible for providing various patient care activities and for maintenance of the Physical Therapy department clinical areas through the performance of various general department duties.
? Performs Physical Therapy evaluations as ordered by a physician.
? Provides for the implementation of proper treatment programs through themselves or through supervision of a physical therapy assistant or aide.
? Periodically evaluates the patient's response to treatment and alters the program as appropriate with the physician's approval.
? Administers standard diagnostic or prognostic tests of muscle, nerve and joint for functional ability.
? Performs therapy services in both the inpatient and outpatient settings and Home Health patients as assigned.
? Maintains appropriate patient records including the initial evaluation, daily treatment charting, progress reports, and discharge summary in the patient's medical record.
? Documentation on the Multidisciplinary Care Plan and Teaching Record.
? Maintains effective relationships with physicians, physician's offices, other departments within the hospital, coworkers and patients.
? Knowledge of the theory, techniques and practices of accepted physical therapy treatments for geriatric, adult, adolescent and child patient populations.
? Administration of physical therapy treatments in the acute care, outpatient and home health care settings.
? Graduation from a Commission on Accreditation of Physical Therapy Education (CAPTE) accredited Physical Therapy Program.
Required Licensure and/or Certifications:
? Possession of a valid license as a Registered Physical Therapist issued by the Board of Medical Examiners, State of California or eligible for license.
? American Heart Association BLS CPR certification.
