Population Health Management (PHM) is an innovative model of care delivery that aims to improve the health of patients while lowering the overall costs of care. PHM includes best practices and tactics to enhance the patient experience, improve outcomes, and provide better organized and more personalized care to our patients.
The Project Specialist will support the day-to-day operational needs for implementation and expansion of programs for PHM Telehealth initiatives. The Project Specialist will provide direct support to community practices by performing test calls for patients scheduled for virtual visit appointments, walking patients through the software installation process, and act as a first line of technical support. Additionally, the project specialist will assist with maintaining project plans and action items, prepare presentations and meeting materials, coordinate program events/learning sessions, manage data and reporting activities, and assist with other special projects as needed. The project specialist will work collaboratively with other members of the team.
PHM Telehealth Programs currently include virtual visits, encompassing both video and e-mail based visitsPRINCIPAL DUTIES AND RESPONSIBILITIES
Assist Program Director with day-to-day tasks and operational activities to support the implementation and expansion of PHM Telehealth programs across the network
Perform test calls with patients and maintain data within the virtual visits application
Walk patients through the software installation process and provide front-line technical support to address any issues that may arise.
Develop and maintain project plans and proactively manage and coordinate open action items
Work collaboratively with clinical and administrative teams at hospitals and practices across the Partners network to support program development and implementation activities
Coordinate data and reporting activities (work with analysts on data requests and independently interpret and present data)
Support the transfer of best practices among Partners entities
Coordinate planning and logistics for all related meetings, educational meetings and demos
Prepare presentations, meeting materials, minutes, and action plans
Stay informed about national developments related to PHM and Telehealth
Develop effective relationships and collaborate with network leaders, physician leaders, and Partners staff
Collaborate with other population health initiatives
Respond to issues raised by stakeholders, practices, and prospects
Provide assistance on other projects as needed
Work collaboratively in a team environment
BS/BA or equivalent required
Masters degree preferred
At least 3 years experience, strongly preferred in a healthcare setting
Strong organizational, administrative, customer service, and project management skills
Ability to prioritize and resolve critical issues efficiently and effectively
Ability to work with all levels of an organization
Strong analytic skills, ability to turn data into information
Ability to effectively present ideas and information
Experience with Microsoft Office applications
Solid understanding of the health care environment
Ability to work independently
Ambulatory practice experience a plus
Position located in Needham, moving to Somerville, with frequent local travel
