Public Health Nurse PDW Spanish Req'd

Congregate Care provides on site care to elderly, disabled and chronically ill residents of adult homes, assisted living facilities, senior citizen housing and naturally occurring retirement communities throughout the Bronx, Brooklyn, Manhattan, and Queens. In addition to coordinating VNSNY services and providing care to individual patients, the nurse based at a designated site serves as a resource for all building residents by providing health information, health screenings, health education, and health linkages. These services help our clients maintain their highest level of health and independence.We are seeking talent for the opportunity described below.
