The Quality Data Nurse is a professional Registered Nurse performing activities related to chart abstraction, analysis, interpretation, audit, verification and improved patient care, process of care and outcomes within the Heart and Vascular Hospital. The position effectively coordinates quality data entered into the HVH database for review and submission to the Department of Health and other cardiovascular registries as determined 1. Data abstraction and input into State and National registries requested by HVH. 2. Analyzes and abstracts medical records against quality indicators or best care measure using complex standard and protocols according to national guidelines 3. Collects, analyzes and reports data regarding core measures 4. Assists with the internal maintenance of databases related to abstracting standard and guideline to ensure accuracy in data entry 5. Assists with data quality control and cleanup of abstracted indicators prior to submission to external sources 6. Monitors and evaluates clinical indicators to meet organizational goals 7. Effectively and efficiently identifies missing data elements in the medical record 8. Assists in maintaining current registry definitions in the HVH Cardiovascular database 9. Plans, coordinates, and implements the Quality Management and Quality Improvement Programs for the associated service line in collaboration with Performance Improvement team when applicable. 10. Monitors and provides assistance with the quality assurance objectives. 11. Assists with chart review and presentation preparation for monthly QA and Morbidity and Mortality Meetings. 12. Provides consultation and direction to ensure programs and services are implemented at the highest standards and patients receive the highest level of care. 13. Attendance at Multidisciplinary Rounds on designated nursing units 14. Assists in the HVH Disease Specific Certifications Working Conditions: Education, Knowledge, Skills and Abilities Required: 1. Bachelor's Degree in Nursing 2. 5 years professional Nursing experience cardiovascular experience preferred. 3. Knowledge of state reporting, data management and quality management principles 4. Ability to work with a multi-disciplinary team 5. Microsoft Word and Excel experience Education, Knowledge, Skills and Abilities Preferred: 1. Proficient Microsoft applications: Excel, Word and PowerPoint 2. Statistical experience 3. Knowledge of team making management practices. 4. Knowledge of Nursing principles, practices, and techniques. 5. Strong computer skills, Excel proficient 6. Skill in both verbal and written communication. 7. Skill in maintaining effective working relationships. Licenses and Certifications Required: 1. NJ State Professional Registered Nurse License. Licenses and Certifications Preferred:
