The work involves the delivery of patient care at the Regional Center of Excellence for Transplant and Kidney Care of the Erie County Medical Center Corporation. The incumbent utilizes the nursing process of assessment, planning implementation and evaluation to provide services for end stage renal disease (ESRD) and home hemodialysis training. Work is performed under the direct supervision of higher ranking professional nursing staff. Supervision is exercised over lower level nursing personnel. Does related work as required.

TYPICAL WORK ACTIVITIES:
Assesses physical, psychological and social dimensions of the end stage renal disease patient;
Assesses patient/family need for instruction;
Educates patient/family about ESRD and self care techniques;
Assists in development of the interdisciplinary short and long term care plans;
Identifies the need for home care or alternatives such as outpatient treatment or extended care facility;
Assesses home hemodialysis patients in the clinic, evaluates progress, responds to patient questions, updates short term care plan, reviews laboratory results and follows up on abnormalities according to protocol
Makes home visits to evaluate the appropriateness of the home setting for performing home hemodialysis and to evaluate the teaching plan and instructions given;
Initiates appropriate health care referral;
Maintains dialysis equipment.

FULL PERFORMANCE KNOWLEDGE, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS: Thorough knowledge of professional nursing principles, techniques and the nursing process; thorough knowledge of end stage renal disease and home hemodialysis; good knowledge of the organization functions, policies, regulations and procedures of the hospital as they relate to nursing; good knowledge of medication administration/pharmacology; good knowledge of aseptic technique and management of infections; good knowledge of the techniques and equipment utilized in home hemodialysis; good interpersonal skills; ability to work on-call; sound judgment; understanding; patience; tact; resourcefulness; initiative; dependability; physically capable of performing the essential functions of the position with or without reasonable accommodation.

MINIMUM QUALIFICATIONS:
A) Graduation from a regionally accredited or New York State registered college or university with a Bachelor s Degree in Nursing and two (2) of registered professional nursing experience in the care of patients receiving hemodialysis; or
B) Graduation from a registered School of Professional Nursing and three (3) years of registered professional nursing experience, at least two (2) years of which included registered professional nursing experience in the care of patients receiving hemodialysis; or
C) An equivalent combination of training and experience as defined by the limits of (A) and (B).

SPECIAL REQUIREMENTS:
Possession of license and current registration to practice as a Registered Professional Nurse in New York State

NOTE: Verifiable part-time and/or volunteer experience will be pro-rated toward meeting full-time experience requirements. Full Time - FT Day Shift
