Job Description
Company: Saint Marys Health Care
Facility: Main Hospital at Grand Rapids
Department: Transplant
Oversees and coordinates care for patients undergoing transplantation for an assigned caseload. Facilitates appropriate care, assists patients and families in finding and negotiating that care. Facilitates appropriate utilization of resources and efficient patient progression through the continuum of health care resulting in the most cost effective and highest quality of Care. Bachelors of Science in Nursing or two years of practice in similar role with transplant specialty certification. Current State of Michigan Registered Nursing License. Three years in nursing
Preferred: Previous Case management experience. Part Time under 40 hours per pay period Days
