The Population Health Care Coordinator, under the direction of the Manager of Quality Improvement, has primary responsibility for care management activities for a specific patient panel. This will include identifying the high acuity patient population and developing, conducting, and monitoring care management processes. The Population Health Care Coordinator will work with the Manager of Quality Improvement to develop this position to best serve the needs of the patient population.
Currently this position will be located at the Forsyth campus in Cumming GA. In the next few months this position will move to its permanent site as a business office in Alpharetta.
Required: Bachelor of Science Degree in Nursing and a minimum of 3 years of experience in a clinical setting. Proficient computer skills, including Microsoft Office (specifically Word and Excel) Excellent oral and written communication skills. Good organizational, analytical, and problem-solving skills. Ability to function effectively with a wide variety of disciplines, including physicians and nurses. A team player that can follow a system and protocol to achieve a common goal. Confidence to communicate and collaborate with other community health care organizations and personnel. Able to participate as part of a team to accomplish an assigned project. Preferred: Previous experience in a Primary Care Setting. Previous experience in case management, care coordination, or patient navigation
Flexibility with office location currently at Forsyth campus but permanent site at either Forsyth, Alpharetta or Atlanta is pending.
