Description

Is a member of the Hospice Interdisciplinary Team (IDT). Provides skilled nursing assessments, planning and interventions for terminally ill adults and children. Rotates on-call. Nursing services are based on initial and ongoing assessment of the patient's needs and are provided in accordance with the IDT's Plan of Care. Assumes primary responsibility for a patient/family caseload. Supports the patient's unique spiritual and cultural beliefs in an effort to maintain the patient's dignity.

Requirements

EDUCATION:

Must be a graduate of an accredited school of nursing.

EXPERIENCE:

Minimum of one (1) year clinical nursing experience with home care experience preferred or as per state regulations.

LICENSURE/CERTIFICATION:

Licensed to practice as an RN in the State of Louisiana or in accordance with the requirements for multi-state licensing, the Nurse Licensure Compact (NLC). CPR Certification. Current Louisiana driver's license, current automobile liability insurance and adequate transportation.

EQUIPMENT/SKILLS:

Well-developed and effective time management skills; effective communication skills (verbal and written); legible handwriting; critical thinking skills; professional appearance and conduct; operation of equipment over performance of procedures; medical--surgical nursing skills; be able to practice independently; strong assessment skills; teaching ability; flexibility; map reading. Computer literacy preferred.

WORKING CONDITIONS/PHYSICAL REQUIREMENTS:

When in the corporate office, works in offices, which are clean, well lit, and air-conditioned. Work may also involve exposure to pleasant and unpleasant working conditions or undesirable elements. May involve exposure to a variety of components that are potentially hazardous or harmful and requires precautions for protection from injury or illness. May occasionally involve exposure to harsh weather and hazardous driving conditions.

OTHER:

Must be knowledgeable of applicable Agency policies, procedures, and organizational structure; must adhere to the policy on Code of Ethical Behavior and our Compliance Plan; must work well with all levels of Associates; must be able to remain calm in the face of irate Clients; and most importantly, must DEMONSTRATE THE CORE VALUES AT ALL TIME.
