We are currently searching for a Reach MTM Pharmacy Technician to work in a progressive dialysis environment where Pharmacists and Pharmacy Technicians provide critical clinical services to patients with chronic kidney disease. The MTM Pharmacy Technician will play an integral role in the provision of medication therapy management services and transitions of care working directly with the Pharmacist and other healthcare disciplines. The overall goal is to work collaboratively as a member of the healthcare team to optimize patient care and improve overall outcomes.
**Essential Duties and Responsibilities:**
Under the direct supervision of a Pharmacist, the Reach MTM Pharmacy Technician's responsibilities include:
+ Facilitate person-to-person telephonic and video-conference based consultations for patients enrolled in MTM program
+ Upload pharmacy related documents into electronic health records as needed
+ Collect, organize, and document patient medication(s), medical diagnose(s), prescription insurance information and provider response(s) in electronic health record or MTM software system
+ Reconcile medication and medical diagnoses between DCI electronic health record and MTM software application
+ Perform complete and accurate technical and clerical duties
+ Data collation and reporting as requested
+ Generate continuous quality improvement reports of progress for each market served;
+ Communicate with other health care team members and insurance company representatives as needed;
+ Provide customer service that makes both internal and external persons feel welcome, important, and appreciated
+ Develop and maintains good relationships with parties for which services are provided
At all times, the Technician must demonstrate ethical conduct and maintain confidentiality of patient information. Performance of these duties will increase Reach MTM program scale and efficiency and enhance patient health and safety.
+ Observes strict confidentiality of patient and personnel information.
+ Conducts self in a professional manner as demonstrated by appearance and attitude, by maintaining a clean and orderly environment, and by dedicating work time to meet patient and DCI needs.
+ Recognizes and responds appropriately to emergency situations.
+ Understands clinic agreements and contracts.
+ Follows the facility attendance policy (absence and tardy).
+ Displays a positive attitude toward patients, co-workers, physicians and DCI.
+ Assists in meeting clinic goals.
+ Performs other duties as assigned.

+ Knowledge of medical terminology, brand and generic names of medications and general pharmacy terminology
+ Ability to perform tasks accurately and efficiently when inputting information
+ Ability to maintain composure in person and while on phone and video when handling calls or participating in video conferences.
+ Intermediate typing/keyboarding skill
+ Attention to detail
+ Ability to use metric system and avoirdupois measures
Education and Experience:
+ High school diploma or equivalent
+ Associates degree from accredited college highly desired
+ Maintenance of State Board pharmacy technician certification in good standing
+ 6 months 1 year of related Pharmacy Technician experience preferred
Certificates, License and Registration:
+ Licensed/registered pharmacy technician or as required by state law

Blood Borne Pathogens
Since this is a healthcare environment, there is the possibility of exposure to infectious/contagious diseases, hazardous chemicals and materials, needle sticks, and blood and body fluids. Further information regarding this position s specific work exposure category will be provided during orientation. This position is classified as Category III.
