Foundations headquarters is located just outside of
Nashville, Tennessee, in the beautiful city of
Brentwood. In the corporate office, you will find FRN s
Marketing, Human Resources, Information Technology,
Accounting, Business Office Services, and Admissions
Center. Each department plays its part in the support of
our facilities, so that the attention can be where it
should be: on the patients. Not only does the corporate

Foundations Recovery Network is currently seeking a
Referral Coordinator, to be responsible for handling all
transferred calls, probing appropriately to qualify the
call, and referring callers to the appropriate treatment
center. The Referral Coordinator will follow up with
each facility to verify admissions status and records
into TIER system and Salesforce. This position will be
located at Foundations Recovery Network s Corporate
Office in Brentwood, TN.

Essential Duties and Responsibilities
Facilitates outbound referrals to aid in placing
patients in the appropriate programs outside of
Foundations Recovery Network
Answers incoming transfer calls, noting telephone code
and caller s number, and obtains caller s name and best
call back number
Utilizes appropriate bonding techniques to establish
rapport and build relationship throughout the call
Qualifies the caller s ability to private pay or
secure insurance information
Utilizes Tier, Salesforce, and the referral grid while
assisting with referrals
Enters or updates record in TIER
Updates Salesforce as required
Follows the defined follow up process based on
information obtained
Maintains communication with the facility, the
patient, their family, or the referral source, all
designed to build a system of opportunities throughout
the call to close process
Demonstrates a strong sense of urgency from initial
call to time of admission, minimizing the number of
calls necessary (preferably one) from initial call to
solid transfer
Efficiently communicates with facilities to follow up
on transferred patients status
Notifies Program Director, nurse manager, and/or other
unit staff of any special medical needs or
accommodations a patient may request or require in order
to evaluate the appropriateness and risk associated with
the admission of the patient
HIPAA Compliance - Must obtain approval from patient
before referring the patient to an appropriate facility
Establishes follow-up contact with interviewed
customers and their referring sources to report outcome
of the process (regarding appropriateness for services)
Knowledge, Skills, and Abilities
Ability to inspire client confidence in the FRN Staff
and the FRN referral process
Ability to consistently demonstrate, through conduct
and communications, the stability required to
effectively assist callers through the FRN Model of
Treatment and the referral process
Knowledgeable of Patient s Rights and treats all
callers with dignity and respect in a consistent, caring
Ability to communicate effectively both verbally and
in writing
Demonstrates a broad base of knowledge in psychiatric
and addiction treatment and administration
Self-directed to provide support to colleagues as
workflow allows
Understands the need for and maintains appropriate
confidentiality at all times when interacting with
patients, residents, families, visitors, referral
sources, and all other contacts
Demonstrates professional behavior and interpersonal
skills reflective of FRN mission statement and
Ability to establish priorities and meet deadlines
Highly organized
Ability to read, write, and analyze detailed and
complex information
Must be able to multi-task between the telephone and
the computer
Must have strong knowledge of Microsoft Office
applications (MS Word, Excel, Outlook)
Education and Experience
High school diploma required; Bachelor s degree
Minimum one year experience in
transactional/relationship customer service; inside
sales or call center experience highly desired
Experience in the alcohol and drug field or
psychiatric treatment field highly preferred
