Referral Coordinator - Hospice

POSITION SUMMARY: The Referral Coordinator is responsible for coordinating all referrals into admissions. This individual will take referrals and coordinate with the clinical Manager and the admission nurse to complete the admission. This person will be responsible for obtaining physician orders, ensuring all information is complete and coordinating the admission. MINIMUM QUALIFICATIONS: Associate or bachelors degree in human services or business field required. Previous sales experience preferred. Customer service experience, excellent verbal communication and listening skills, excellent organization skills and attention to detail, healthcare experience preferred, strong time management skills, task oriented while conveying compassion and empathy to patients and families, flexibility with the ability to multi-task to accommodate changing scheduling and admission needs through the work day, writing and computer data entry required.
