Role Statement:

Responsible for coordinating all aspects of the patient intake process including accepting incoming referrals, updating referrals and marketing Virtua Home Care to incoming calls about services provided. Collects accurate information and accurately documents on all referrals including services needed, patient history, Face to Face, encounter information and home environment. Supports On- Line referral process. Assists with identification of community physician (PCP) collaborates with physicians to obtain Home Care orders. Establishes and maintains a positive relationship with customers and referral sources, responding to customer requests and concerns in a timely manner. Actively engages and collaborates with other team members to maintain a positive work environment. Demonstrates and maintains high standards of professional ethics.

Job Essentials:

-Ensures seamless transition of patients to home care by obtaining and documenting accurate referral information

-Effectively manages rapidly changing volume within the department

-Collects accurate insurance authorizations as needed

-Maintains comprehensive working knowledge of community resources and assist referral sources in accessing community resources should services not be provided by agency

-Participates in orientation, education and development of the healthcare team

-Identifies and participates in performance improvement activities

-Maintains professional and departmental level competencies

Educational Requirements, Qualifications:

Must have valid NJ RN license. BSN preferred, 2-3 years Home Care experience preferred. Approved licenses must be presented and maintained in accordance with the State of New Jersey Licensing Board. Refer to the Educations/Regulatory Requirements (Department Specific) policy in the Nursing Adminstrative Manual. Demonstrates knowledge of nursing skills, hospital and home care practices, procedures and standards. Perfects skills and stays abreast of current nursing practice through on-going education, journals, ect.
