Summary of Position:

A Registered Nurse employed by Case Management to maximize coordination of care within the continuum of care. The Manager of Case Management works collaboratively with medical, nursing and other health care team members to assure that services flow continuously from assessment through treatment and reassessment. The Manager of Case Management oversees the process of coordinating the plan of care to avoid duplication of services, makes referrals to appropriate community resources, provides education to prepare the patient for discharge referrals or transfers patients according to the needs of the patient and facilitates communication and coordination among all members of the health team. The Manager of Case Management assists with administrative duties in order to back up the Director and to place themselves in line for promotion. Performs the role of Director of Case Management in his/her absence. The Manager of Case Management oversees, mentors and assists the front line employees in the department. Must follow and adhere to all federal and state laws and regulations, as well as the HCA Code of Conduct and Rio Grande Regional Hospital Performance Standards.

Educational Level:

Master s degree or advanced degree program, MSW or MNS
Bachelor s Degree

Licensure/Certifications:
RN or MSW

Experience:

A Manager of Case Management is responsible and accountable for developing and monitoring the clinical/quality and financial outcomes of a patient caseload. The Manager of Case Management is knowledgeable in the role of case management and is the mentor to others. Previous Case management/ utilization review required. Employee must be able to demonstrate the knowledge and skills necessary to provide care or support based on physical, psycho/social, educational, safety, and related criteria, appropriate to the age of the patients served in his/her assigned service area, as defined in this job description/performance evaluation.
