Duties and Responsibilities
The Licensed Clinical Social Worker (LCSW) will provide quality clinical interventions to patients in need of behavioral health services, including counseling for alcohol and substance abuse. The LCSW will work within a primary care clinic as an integrated team member and provide consultation to providers and staff concerning behavioral interventions to improve patient functioning and health status. This position is funded by a HRSA grant investigating the efficacy of behavioral health treatment within primary care.
Full time position in a primary care clinic.
Some weekday evenings required.
Half day Saturdays required monthly.
-Conducts initial evaluations of referred patients, focusing on diagnostic and functional abilities and development of shared treatment goals;
-Provide primary care providers and interprofessional collaborative team with relevant feedback regarding clinical recommendations and treatment plans through documentation in the electronic medical record and verbal feedback;
-Provide ongoing, focused interventions tailored to needs of patients;
-Develops treatment plans to prevent relapse and communicates these plans to primary care providers, team and patients;
-Advise primary care providers and team members regarding specialty area of alcohol and substance abuse and behavioral health problems;
-Advises primary care providers and patients regarding appropriate level of care including referral to psychiatry or more intensive treatment programs;
-Timely documentation of patient progress, diagnostic information and treatment plan within the electronic medical record;
-Participates in research supporting the effectiveness of behavioral health integration in primary care including monitoring patient progress and outcomes;
-Develop and conduct group behavioral health interventions based on patient population needs within the primary care clinic;
-Provide interventions across the life span, including children, adolescents, adults, and families;
-Communicate effectively with patients regarding how to make behavioral changes and maintain healthier lifestyle choices;
- Provide patients with self-management skills and educational information to enhance coping;
- Provide therapeutic interventions to treat the wide range of emotional and cognitive issues that patient's present within a busy primary care clinic;
-Help patients decrease targeted alcohol and substance use whether through provision of individual psychotherapy, group or referral to appropriate outside treatment programs and facilitate entrance into those treatment programs;
-Provide therapeutic interventions to help patients cope with pain; acute and chronic medical conditions; and encourage adherence to medical treatment plans;
-Develop and participate in health promotion interventions with community partners and other members of interprofessional team;
-Mentor and supervise Social Work and other health care professional students;
-Teach faculty, staff, residents, and medical students about substance abuse screening, referral and interventions;
-Practice in accordance with federal and state regulatory standards.
Minimum Education or Experience
Master's degree in social work and one-three years of related experience. Licenses: (LCSW) in Illinois
