JOB DESCRIPTIONSHORT DESCRIPTION:
Demonstrates skills and knowledge necessary to assure assessment, treatment and care appropriate to the age of the patients served. Conducts comprehensive evaluation procedures for patients with known or suspected communication problems. Documents results of evaluation procedures in report forms. Conducts individual or group treatment sessions to remediate communication disorder. Responsible for implementation of follow-up procedures for patients receiving speech and language services. Must prepare monthly Medicaid and Medicare recertification plans containing long term and short term objectives. Discharge summary must be written on outpatients, and inpatient discharge note written in progress notes.
Scheduled Days / Hours: Monday-Friday; Current NJ-SLP - NJ Speech Pathologist License.
