Social Service Coordinator

Description:
Celebrate Caring at Forest Hills Care and Rehabilitation
Center, a SunBridge Healthcare affiliate. We are
currently seeking a full time Social Service Coordinator
at our 149-bed center located in Broken Arrow, OK.
Primary responsibility is to assist the Social Services
Department under the direction and supervision of the
Director of Social Services or Social Services
Consultant in accordance with current guidelines and
regulations; and established policies and procedures.
The focus is to assist each resident and family in
adjusting to placement, illness, and plan of care to
attain the highest level of functioning.
Requirements:
* A related degree and/or experience that fulfills state
requirements (in applicable states) for Social Service
Designee; Social Work certification/license in
accordance with state and local standards.
* One-year experience working with a geriatric
population or in a long-term care setting
preferred.
* Strong interpersonal and communication skills.
* Ability to make independent decisions and to follow
instructions.
* Ability to manage multiple tasks and projects.
* Ability to participate in and lead Interdisciplinary
Teams.
* Ability to respect residents including those who have
impaired cognition.
