Provides social work and discharge planning services including psychosocial assessments, brief patient/family counseling, support, resources, education and intervention. Arranges and communicates efficient, timely and cost-effective discharge plans.Current unencumbered RN license or Bachelors degree in social work, psychology, education or a related field required.Experience and/or training in health care, health care delivery systems, managed care and case management (including Medicare and Medicaid) required.Experience as a Social Worker, Nurse, Discharge Planner or Case Manager in a hospital or medical setting required.Experience and/or training in crisis intervention and counseling regarding a wide variety of medical, social, emotional and financial difficulties requiredExperience and/or training with computer applications required.
