The Evening/Night Shift Care Coordination Social Worker for Emergency Patient Resources and Options (EPRO) provides specialized telephonic care coordination, suicide risk assessment and crisis intervention services, including consultative social work support to a multi-disciplinary medical team. Provides telephonic crisis support for the purpose of enabling the patient to maintain the greatest level of independence and safety. Coordinates with others to establish a safe plan of care. Provides short- term counseling and crisis intervention, teaching and brokering with other community and Group Health resources to meet care coordination needs. Minimum Education Requirements
Masters of Social Work (MSW).
Additional coursework in Chemical Dependency
MINIMUM KNOWLEDGE/SKILLS REQUIREMENTS
Excellent written and verbal communication skills. Demonstrated success as part of a multi-disciplinary team and commitment to diversity.
Knowledge of medical social work case management and care coordination, suicide risk assessment, crisis intervention and community resources. Strong interpersonal and clinical skills. Ability to deliver telephonic crisis social work services. Familiarity with Microsoft Office. Familiarity with electronic charting.
MINIMUM EXPERIENCE REQUIREMENTS
1 year Previous social work experience
2 years Post-Masters experience working with mentally ill and/or chemically dependent individuals. Care coordination and Crisis Social Work experience in an emergency department or acute mental health setting performing suicide risk assessments, safety planning and psychiatric bed placements. Prior experience working with managed care and/or community mental health systems.
MINIMUM LICENSE/CERTIFICATIONS REQUIREMENTS
At time of hire Washington State Licensed Social Work Associate (LSWAA or LSWAIC) or Licensed Advanced Social Worker (LASW).
PREFERRED LICENSE/CERTIFICATIONS Licensed Independent Clinical Social Worker (LICSW).
