GENERAL SUMMARY

Compassionate and dedicated Social Worker to provide social services to the hospice/palliative pediatric patient and their caregivers to promote psycho-social, emotional, functional and physical well-being throughout the continuum of care. Also responsible for the development and administration of the Hospice Volunteer Program, to include recruitment, training, and supervision of volunteers as well as prepares and maintains reports/statistics as required.

MINIMUM QUALIFICATIONS
1. Masters degree in Social Work
2. State Licensure required; LISW preferred.
3. Demonstrates skills in managing care to children and families.
4. Excellent communication and interpersonal skills to establish and maintain positive relationships with patients, families and community professionals.
