Facility: Laurel Regional Hospital (236-RJZ) at Laurel
Department: Mental Health Services
The Social Worker-Total Care Manager will coordinate and monitor care for assigned patients, facilitating patient movement throughout the continuum of care. This position assumes a leadership role within the multidisciplinary team to achieve quality, clinical and financial outcomes. Assigned duties include completing a psychosocial assessment within 24 hours for new clients, coordinating the plan of care established by the treatment team, identifying and arranging appropriate discharge plans, and providing support and resource information to patients and families.
Minimum Qualification:
Masters degree in Social Work (MSW) from an accredited program
Licensed social worker in the state of Maryland
One (1) year experience in a medical setting as a social worker or social work associate, and two (2) years of case management experience
Excellent oral and written communication skills
Intermediate proficiency with MSN office
Full Time Days Masters Degree MD State Licensure required 2-3 years experience required
