Facility: Upper Chesapeake Medical Center at Bel Air
Department: Palliative Care - HSP

JOB SUMMARY: Provides social work and discharge planning services for palliative care
inpatients accessing care through Upper Chesapeake Health. Screens patients to identify
anticipated needs. Works collaboratively with inter-disciplinary team to assess plan of
care, to monitor progress and to interact with patient and family so that the safest possible
placement is achieved. Coordinates and implements discharge and post-acute services for
inpatient caseload. May interact with patients or customers ranging in age from newborn
to geriatric.




Education & Training: BSW from a school of Social Work accredited by the Council of Social Work Education required. Maryland licensure at the LGSW level preferred.

Work Orientation & Experience: Acute care social work experience preferred. Knowledge of Palliative Care, Case Management, Critical Pathways, and Utilization Management preferred.
Skills & Abilities: Demonstrate skill in: a) social work casework; b) assessing patient and family dynamics; c) developing and implementing discharge plans; d)brokering services for patient/family with vendors; e) effective critical thinking; f) written and verbal communications; g) prioritizing work assignments and managing time effectively to complete duties; and h) age appropriate interpersonal interactions (patients may range from newborn to geriatric adult). Demonstrate knowledge of: a) Medicare and Medicaid regulations and documentation and b) community resources and c) case management principles. Demonstrate ability to a) communicate and collaborate effectively with both internal and external customers (colleagues, Medical Staff, liaisons, and patient/family); b) assess, adapt, and calmly respond to changing and/or crisis environment; c) make independent decisions consistent with current policies, procedures, health care and ethical standards; and d) be available to adapt hours to meet the needs of all customers.
