Provides supervision and clinical oversight to staff to ensure consistency of care coordination processes. This includes planning, coordinating, and monitoring the safe transition of patients from acute or subacute care to home, as well as orienting the patient in the community with home health services available within VNSNY Programs. Provides oversight of clinical documentations of patient data that will ensure quality transcription of referral details. Provides support for customer service staff to ensure quality customer service to internal and external customers. Works under general direction.

Licensure: License and current registration to practice as a Registered Professional Nurse in NYS required.

Education: Bachelors Degree in Nursing required. Masters Degree preferred.

Experience: Minimum of four years experience in community health, health care administration, LTHHC or discharge planning required. Minimum one year management/supervisory experience required, preferably within intake staff or transitional care. Strong clinical knowledge with understanding of regulatory requirements that govern home care practices, policies and reimbursement required. Strong group facilitation skills required. Demonstrated leadership ability and knowledge of the business required. Good computer skills and proficiency with Microsoft Office (i.e. Word, Excel, and Outlook) required.
